Describe in detail what happened including the concern or potential issue, the environment (weather, terrain, fire behavior, etc), and the resulting health issue.
Radio communication plan was agreed upon and finalized. The radio tech cloned all of the radios for the forest. During our lightning event and initial attack we had noticed we were not hearing everyone on the radio. The field also realized they were not able to reach us on certain repeaters. At first we thought it was a network error due to having many network issues and the radios going in and out. The crew actually figured out it was a radio clone issue. More than 20 mistakes were cloned into all the radios on the forest.
Some of the issues found; Transmit code guard was wrong, the receive frequency and the code guard wrong on one of the main repeaters, incident tac wrong and this is consistent with about 20 plus channels and effected all groups in the communication plan.
File that is attached was the first round of mistakes found. Then fire personal looked at all the frequencies and found others.
Immediate Action Taken

The radio tech was contacted. It was made clear what the problem was by the fire personal. A meeting was set up to go over all the issues the next morning. Once the issues were identified a quick clone was done to get one group correct and the others are planned to get fixed over the weekend. Once all the mistakes are corrected another forest wide clone will be completed. The radio tech will work through the weekend to get this done and re cloned before our next lightning event this up coming week.
