Describe in detail what happened including the concern or potential issue, the environment (weather, terrain, fire behavior, etc), and the resulting health issue.
Denial of drinking water, unqualified personnel in battalion chief positions, as well as in the captain positions either certifications, expired, or nonexistent. Hostile work environment. Overhead ordering engines in front of fire head lack of communication, cloning equipment, such as cloning cables and the adapters for new radios. Mobile radios unusable due to improper installation. Most of the trucks are overweight and weighed dry. Sexual-harassment reports being swept under the rug can provide way more documentation as needed.
